normal align=left>二、投标申请人资格要求：
normal align=left>参加竞标的投标人必须满足投标资格要求中的所有条款，并按照相关规定递交资格证明文件。
normal align=left>1、投标人必须是中华人民共和国境内注册，具有独立法人资格；其经营范围应包含：医疗器械的生产（制造商）或销售（代理商）。
normal align=left>2、投标设备在国内销售没有不良记录、没有发生过重大质量问题或安全事故；
normal align=left>3、投标人必须是所投产品的制造商或代理商。
normal align=left>4、本项目不接受同一品牌的两个及以上投标人同时投标，开标后，若出现同一品牌的投标人有两家以上，那么对同一品牌的所有投标人予以废标。
normal align=left>5、投标货物必须符合国家标准、行业标准和专业标准等相关标准，并符合中华人民共和国国务院令第650号修订后的《医疗器械监督管理条例》相关规定。
normal align=left>6、生产企业须具有医疗器械生产许可证；代理商（经销商）须具有医疗器械经营许可证。
normal align=left>7、具有良好的商业信誉和完善的售后服务体系，并能承担采购项目供货能力和服务。
normal align=left>8、企业注册地或项目所在地检察机关出具的无行贿犯罪记录证明。
normal align=left>9、投标人必须满足对投标资格的其他要求。
normal align=left>10、实质响应招标文件提出的关键技术参数等要求。
normal align=left>11、本项目不接受联合体投标。
normal align=left>三、招标文件的获取：
normal align=left> 本次招标项目取消现场报名，凡符合上述要求的投标人请于2017年1月18日至2017年1 月 23 日登陆安阳市公共资源交易中心网站（www.ayggzy.cn），完成用户注册后下载招标文件。
normal align=left>四、请投标人注意：
normal align=left>各潜在投标人请到安阳市公共资源交易中心办理数字证书认证，办理及使用流程请到安阳市公共资源交易中心网站资料下载栏中下载（医疗设备系统使用说明—>医疗设备网上投标系统操作文档）。
normal align=left>办理地址：市民之家二楼西厅31号窗口。请在工作时间内办理数字证书，以免影响正常投标。工作时间：上午9：00—12：00，下午 1：00—5：00

normal align=left>招标文件售价：300元/份，售后不退。开标时交纳。
normal align=left>五、投标文件的上传及开标时间
normal align=left>5.1投标文件上传的截止时间及开标时间为2017年 2 月 14 日 上午10:00整地点为安阳市民之家四楼公共资源交易中心第 五 开标室。
normal align=left>5.2 开标截止时间前投标人须将电子投标文件上传完毕，否则招标人不予受理。
normal align=left>5.3电子投标文件递交方式：
normal align=left>网上递交：投标人必须在办理数字证书后在投标截止时间前完成所有投标文件的上传，逾期将无法上传电子文件。
normal align=left>六、招标公告发布地址：
normal align=left>本招标公告同时在《中国采购与招标网》、《河南省政府采购网》、《河南招标采购综合网》、《安阳市政府网》、《安阳市公共资源交易中心》网站上发布。
normal align=left>七、注意事项
normal align=left>7.1 投标人获取文件后，如对招标文件内容有质疑应在2017年 2 月 4 日下午16:30 前以不记名形式上传至安阳市公共资源交易中心网站。
normal align=left>7.2本次招标项目如有变更或延期，投标人均可在安阳市公共资源交易中心网站直接下载补充文件，投标人应随时关注安阳市公共资源交易中心网站。
normal align=left>7.3开标后由评标委员会对投标单位的资格证明材料进行资格审核，不符合项目资格条件的投标单位的投标将被拒绝，投标单位应自负风险费用；提供虚假材料的将进一步追究其责任。
normal align=left>7.4特别说明：电子投标文件通过网络上传成功即视为签到成功，投标文件因网络原因无法正常上传，或电子文件格式不正确无法打开上传文件的，视为无效标，现场不接受纸质版投标文件，同时现场不接收u盘上传标书文件。
